This policy explains what information All Smiles General Dentistry (“we,” “us”) collects through this website, allsmilesgeneraldentistry.com, how we use it, and the choices you have. It covers the website only.
Your dental records are covered separately
Information in your patient record — exams, X-rays, treatment and billing — is protected health information under HIPAA. How we use and share it is described in our Notice of Privacy Practices, which is also available at our office and on request at (305) 893-4801. Information you give us
When you use our appointment request form, we collect your name, email address, phone number, whether you are a new patient, whether you have dental insurance, and anything you write in the message box. We use it to call or email you back about your request. The form is built with WPForms, and submissions are stored on our website and emailed to our office.
Please don’t include detailed medical information, Social Security numbers or insurance ID numbers in the form. We’ll collect what we need securely when we speak with you or at your visit.
If you call us, we use the information you share to schedule and prepare for your appointment.
Information collected automatically
- Analytics. We use Google Analytics to understand how visitors use the site — for example, which pages are viewed and how people arrive. Google Analytics uses cookies and collects information such as pages visited, approximate location, device and browser type. We do not use it to identify you.
- Google Tag Manager. We use Google Tag Manager to load the analytics and advertising-measurement tags described here. It does not collect information on its own.
- Advertising measurement (Google Ads). We advertise on Google. When you arrive from one of our ads, a Google Ads tag records whether that visit led to a phone call from the site or an appointment request, so we can tell which ads are useful. Google may use cookies or similar identifiers for this and, depending on our account settings, to show our ads to people who have visited this site. We do not send the message you write in our form to Google. See how Google uses information from sites that use its services.
- Spam protection. Our form uses Google reCAPTCHA to block automated spam. reCAPTCHA collects information about your device and how you use the page, under Google’s Privacy Policy and Terms of Service.
- Server and security logs. Our web host and security software record technical details such as IP address, browser and the time of each request, to keep the site running and protect it from attacks.
- Embedded map. Pages with a Google Map load it from Google, which may set its own cookies under Google’s privacy policy.

- To measure which of our online ads lead to calls and appointment requests
- To operate, secure and improve the website
- To understand which pages and services are most useful to visitors
- To meet legal obligations
We do not sell your personal information, and we do not use what you type into our form for advertising.
Who we share it with
We share website information only with service providers that help us run the site — such as our web host, our email delivery service and Google (analytics, advertising measurement, spam protection and maps) — and only as needed for them to provide those services. We may also disclose information if the law requires it or to protect the rights and safety of our patients, staff or others.
Cookies and your choices
- You can block or delete cookies in your browser settings. The site will still work.
- You can opt out of Google Analytics with Google’s Analytics opt-out browser add-on.
- You can limit personalized ads from Google in My Ad Center.

How long we keep it
We keep form submissions only as long as needed to respond to you and for our normal business records. Information that becomes part of your patient record is kept as required by Florida law and HIPAA.
Security
The site uses encrypted connections (HTTPS), and we limit access to submitted information to staff who need it. No method of transmission over the internet is completely secure, so please use the phone for anything sensitive.
Children
This website is intended for adults. Parents and guardians may request appointments for their children. We do not knowingly collect information online directly from children under 13; if you believe a child has sent us information, call us and we will delete it.
Links to other sites
Our site links to other websites, such as Google Maps, Facebook and Instagram. Their privacy practices are governed by their own policies.
Changes to this policy
We may update this policy from time to time. The effective date at the top shows when it was last changed.
